Interview with a specialist in dermatovenerology
A specialist in dermatovenerology is an expert who knows how to prevent premature skin aging and how to prevent or manage skin diseases.
We visited her at Terme Dolenjske Toplice, at the Aesthetic and Dermatological Clinic.
What are the biggest enemies of the skin?
The skin is an extremely important organ that protects the inside of the body from harmful external factors. At the same time, it is our mirror, revealing our lifestyle and overall well-being. It is therefore essential to protect the skin and take good care of it. The problem, however, is that the consequences of harmful influences on the skin do not appear immediately, but only over time. One of the most common causes of skin damage is excessive sun exposure, for example, with consequences often becoming visible only decades later.
The effects of smoking, which causes tissue deoxygenation—that is, a lack of oxygen in the skin—manifest as increasingly grey, dull skin over the years. Due to the repeated pursing of the lips while smoking, very unpleasant wrinkles form around the mouth. These so-called “smoker’s lines” are among the most disturbing wrinkles on the face.
The skin is also negatively affected by insufficient sleep, prolonged stress, an improper diet, incorrect skincare—such as squeezing pimples and using unsuitable creams—and excessive application of cosmetic products.
“At eighteen, many people may think: ‘Oh, I don’t care what I’ll look like at fifty.’ But I would like to point out that many of my patients deeply regret not taking better care of themselves when they were younger. At fifty, they are still full of life, active, and want—and need—to be fit and youthful. Prematurely aged skin therefore represents a major problem for them. Many, for example, say they are bothered by a tired and sad appearance that does not reflect how they actually feel.”
Expose 30% of unprotected skin to the sun three times a week for 15 minutes
This is feasible from spring to autumn. In winter, we recommend taking vitamin D supplements. Only in older individuals (over 70 years of age) do we recommend vitamin D supplementation even in summer, as the skin’s synthesis of this vitamin is reduced to one quarter. In winter, we recommend a daily intake of 20 micrograms of vitamin D for everyone, in the form of tablets or foods (fish, fish oil, fortified milk), with a maximum of 100 micrograms per day.

What is excessive sun exposure and what does it cause?
Excessive sun exposure refers to any sunbathing that causes redness, as red skin already indicates a burn or an acute inflammatory response. Although the sun also has positive effects on the body, people often overdo sun exposure—either in pursuit of a tan or due to carelessness or lack of knowledge. As we know, any excess is harmful.
A tan can also be achieved gradually and in a safer way—it is important to leave the sun immediately once redness begins to appear. In this way, we give the skin time to produce additional melanin, the pigment that protects skin cells. One of the most dangerous late biological consequences is skin cancer. It does not develop in the year of the sunburn or the following year, but much later.
And it’s not only about skin cancer—excessive sun exposure also accelerates skin aging, causing scales, spots, pigmentation changes, and loss of radiance. The consequences of mistakes made in childhood thus become visible, for example, in one’s forties or fifties.
I would like to emphasize that we currently do not have laser or other treatments capable of restoring the skin to the condition it would have had without sun damage. Therefore, the best method for maintaining beautiful, healthy skin is protection.
“I often advise my patients to compare the skin on the inner thighs, which is not exposed to the sun, with the skin on the hands, face, or décolletage, which is most exposed. The difference is always obvious.”
What are the signs of premature skin aging?
The skin loses tone and elasticity, becomes loose and sagging, and wrinkles appear earlier due to damage to collagen fibers. Persistent spots and scaling are disturbing, and various skin growths may appear, indicating that the skin is no longer healthy. These signs certainly appear earlier in those who work outdoors unprotected, exercise or sunbathe frequently, and in smokers.

Women after the age of fifty?
Age is just a number, yet the stigma of being “over fifty” is unfortunately very strong, especially for women. It is as if at 50 they are placed into a separate category of ‘old,’ regardless of how they live or look. Why shouldn’t a woman in her sixties be youthful, fit, vital, full of life, beautiful, even sexy? If she takes holistic care of herself—not only her skin—this is entirely possible. Age is also reflected in posture, mobility, mental vitality, energy, and inner spark. This perspective deserves more emphasis.
I am very pleased that Terme Krka places great importance on this and implements a broader concept of health. Smaller aesthetic and other clinics cannot achieve this on such a scale.

What does beauty and a healthy aesthetic appearance mean to you?
I like to emphasize that a healthy, well-groomed appearance is today already part of personal hygiene. At fifty, it is a pity if a person looks prematurely aged, as they are still in a very active stage of life. Especially when we know that technologies for slowing down aging are now so advanced and procedures so effective, accessible, and non-invasive that at fifty one can easily look the way our parents did at forty.
